Roles & Responsibilities Prioritize and complete Start of Care visits promptly to ensure patients receive timely assessments Supervise and mentor Physical Therapy Assistants to deliver high-quality home health care Report Start of Care visits the next day and maintain open communication with your supervisor and intake Complete clinical documentation (Kinnser) within 48 hours of each visit to keep patient records current Manage a patient caseload that averages 8 hours of visits per day while maintaining quality outcomes Use issued tools (iPad, Verizon One Talk) effectively to coordinate care and document visits Qualifications Active CA PT License (required) Minimum 1 year of Home Health experience (required) Experience with Start of Care visits and OASIS documentation Comfort supervising Physical Therapy Assistants and collaborating with clinical teams Familiarity with Kinnser or similar home-health documentation systems preferred Knowledge & Skills Strong clinical documentation skills and working knowledge of OASIS Ability to prioritize and conduct efficient Start of Care visits Time management and caseload planning for community-based care Clear communication with supervisors, intake, and interdisciplinary teams Competence using mobile devices and telephony tools for field documentation and communication Benefits Comprehensive 3-day onboarding (one in-house day two field days) so you hit the ground running Equipment provided — iPad and Verizon One Talk to help you stay connected on the road Mileage, parking and toll reimbursement to cover travel between patients (details provided during onboarding) Company car option available for patient visits Workable, predictable schedule averaging 8 hours/day and 40 hours/week Clinical supervision and clear communication channels with intake and office teams Meaningful work with autonomy to supervise PTAs and manage patient care in the community Opportunity to grow your territory across Solano County Travel Perks: Explore Oakland, CA This contract role is based in Oakland with coverage in Solano County — a great base for Bay Area exploration.
